Amb Care Practices in ND?

    Hello everyone and Happy New Year!

    I'm hoping to jump start some conversation in the Ambulatory Care Practitioners Forum and wanted to get an idea of how many ambulatory care practitioners we have!  So, if you would be so kind to chime in on this question:

    How are you involved with ambulatory care in ND? What is your practice setting? What clinical services do you provide?

    Ambulatory care involvement in ND - I'm one of the pharmacists practicing at the Sanford Southpointe Clinic in Fargo, ND. There are 5 pharmacist that rotate into this position. The position is 1 FTE.  

    Practice setting - Clinic. We work primarily with internal medicine physicians and their patients. 

    Clinical services - We have a collaborative practice agreement for diabetes management.  We are also doing transition of care calls and med rec.

    Great to hear of the services you are providing at Sanford!
    Thought I would share my practice as well:
    I am practicing at Family HealthCare clinic (Family medicine community health center, FQHC setting) in an office-based practice, working with the family med physicians, nurse practitioners and PAs.  We are also a level-3 PCMH.
    I am currently the only clinical pharmacist practicing in the office-based practice, 0.5 FTE.
    I have a collaborative practice agreement for management of drug therapy for a variety of disease states, including anticoagulation, tobacco cessation, diabetes, asthma/COPD, hypertension, hyperlipidemia, CHF, allergic rhinitis and GERD.  I host APPE students who also participate in clinical services, which includes prospective review and recommendations for ASCVD Risk.
